Home Care Packages

Have your needs increased?

Eligible clients will have had an ACAT assessment through My Aged Care and been allocated a HCP (Levels 1 to 4). Further to this, clients will need to contact Centrelink to determine any out of pocket expenses incurred by accessing this package.

Home care packages are based on consumer choice. Participants, in collaboration with the our coordinator create a package of care based on participant’s priorities, budget, and ability, to maintain health and wellness at home.

Commonwealth Home Support Programme (CHSP)

The CHSP funds a range of services that support older people and their Carers to be more independent at home and in the community. Older people are those aged 65 and over and Aboriginal and Torres Strait Islander people who are over 50 years of age.

To be eligible for CHSP subsidies services you must:

  • Be aged 65 years and over OR Aboriginal and Torres Strait Islander aged 50 years or older

  • Live in a state or territory (except Victoria and Western Australia)

  • Be at risk of premature or inappropriate admission to long term residential care

  • Be a Carer of an older Australian eligible for services under the Commonwealth Home Support Program

Queensland Community Care Services

The Queensland Community Care Program funds a range of services that assist people who are under 65 years of age and Aboriginal and Torres Strait Islander people who are under 50 years of age with activities of daily living. The program also supports the Carers of these people.

Veterans’ Home Care (VHC) & Community Nursing Services

Veterans Home Care is funded by the Australian Government Department of Veterans' Affairs to provide a range of basic services to war widows and widowers and Australian Defence Force veterans.
